10/16/2023 EnchantaVaughn Approved Behavior: QMHPA arrived to the home of Ms. Broadnax. Upon arrival to the home setting, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her day. Ms. Broadnax indicated her day was well. Ms. Broadnax was well groomed and prepared for the session. Intervention: QMHPA began the session by inquiring about Ms. Broadnax current thoughts, feelings, and emotions. QMHPA encouraged practicing relaxation training this week in order to reduce anxiety attacks. QMHPA demonstrated one way she can decrease her stress using a stress ball as one tool readily available for her use. QMHPA supports coping with symptoms and problems related to self-afflicted stress by discussing with her, her current/past family related matters and how to prevent dysfunctional family members from disabling her need to remain positive despite occurrences. QMHPA taught Ms. Broadnax adversity management techniques to reinforce self-esteem, self-reliance and assertiveness to accomplish goals without looking backwards on past failures. QMHPA provided behavior management/modification to Ms. Broadnax by addressing the nature of change in accordance to growth and development. QMHPA discussed and instructed Ms. Broadnax with motivational quotes to uplift self-esteem and sense of purpose. QMHPA encouraged developing coping skills to help decrease symptoms and moody behaviors related to excessive worry and self-pity. QMHPA provided recovery strategies for Ms. Broadnax regarding moving forward after a loss and looking forward to a positive future without reliving negative past experiences. Response: Ms. Broadnax appeared to be receptive to QMHPAâs interventions and services that were provided to her. Ms. Broadnax demonstrated a few inappropriate social interaction skills while on todayâs outing but was able to have some optimistic periods in regards to achieving treatment goal objectives. Ms. Broadnax appeared receptive towards some interventions, and recommendations provided to improve self-confidence and create a incentive towards achieving treatment goal objectives daily. Ms. Broadnax acknowledged that she was able to solve some disputes with her family members so that she can move past some of their issues. Ms. Broadnax was commended. Ms. Broadnax expressed to QMHPA that personal circumstances and her failures in life often leave her hopeless and unwilling to try. Ms. Broadnax admitted that her moodiness causing her to become angry and annoyed. Ms. Broadnax shared that she has made a lot of her situations up setting due to being angry, moody and frustrated. Ms. Broadnax shared with QMHPA that her anxiety may raise when she feels compelled by daily life responsibilities or the fact that she hasnât been able to resume feeling normal since the incident. Ms. Broadnax acknowledged that talking to her QMHPA with whom she can trust is a release for her and allows her to vent out her anxiety Plan: QMHPA will continue to teach Ms. Broadnax how to identify as well as process triggers and educate her regarding positive coping skills and interpersonal skills 1-2 times (Weekly).
